NAVIGATION Health Program

Your health is our program!

Health and living conditions are connected. Stress negatively affects well-being, no matter where it comes from: problems at work, with family, or with authorities – all of this can harm your health. Some complaints persist or keep coming back.

The NAVIGATION health program supports you in finding solutions: for physical problems, social challenges, and psychological stress. Because: there are solutions to most problems. We have a team of doctors, specially trained nurses (Community Health Nurses), psychologists, and social counselors. This team works with you to help you navigate the healthcare system and get help.

Registration: How to participate

You can participate in the NAVIGATION health program if these criteria apply to you:

  • You are at least 18 years old.
  • You have persistent or recurring complaints.
  • You have statutory health insurance.
  • You live in Berlin Neukölln (postal codes: 12043, 12045, 12047, 12049, 12051, 12053, 12055, 12059)

متى؟

Registration for the NAVIGATION health program is possible at any time during the project period from April 2025 to October 2026. Your individual support then runs for six months.

Community Health Nurses (CHN)

What is your health situation today? How could it improve? A CHN develops the health program together with you and provides advisory support. We stay in close contact and support you with practical matters – for example, booking appointments or managing medication.

General Practitioner

To address your chronic and acute illnesses, you will receive general medical care. Based on the diagnosis, a general practitioner advises you on treatment options and starts therapy. When the six-month health program is over, we continue to be there for you and continue treatment if needed.

Social Worker

Do you have financial worries, problems with authorities, or residency questions? We advise you on your options and tackle problems together. With us, you learn what rights you have and what you are entitled to. We also help with filling out forms, overcoming language barriers, and writing letters.

Psychologist

Nobody is free from worries. When psychological stress becomes overwhelming, it often helps to talk. In psychological counseling, we listen to you and search together for ways to deal with difficult situations. Our conversations aim to help you gain clarity and continue with inner strength.

More information about the project:

NAVIGATION is an Innovation Fund project of the Federal Joint Committee, which aims to establish primary care centers as a new local form of care in Germany. The project is supported by a consortium of seven, including AOK Rheinland/Hamburg, AOK Nordost, Charité – Universitätsmedizin Berlin, Frankfurt University of Applied Sciences, and the Hamburg Association for Health Promotion. The project management lies with Poliklinik Veddel in Hamburg and GeKo - Community Health Center Neukölln in Berlin. The project runs from August 2024 to October 2027.
